EGD Instructions (Esophagogastroduodenoscopy) Patient Name: ________________________________________ Report to one of the following Florence Hospitals at __________ o'clock Mcleod Hospital, Women's Pavilion, 1st Floor, “Outpatient check-in” ____ Carolinas Hospital System, 1st Floor, “Outpatient Check-in “ ____ You are scheduled to have the actual procedure in the Endoscopy Lab  ON ______________________ AT_________________A.M. (But check-in one hour before the procedure) Read Carefully 1.Take this sheet with you to the hospital on day of procedure. 2.Preregister at “outpatient check-in” at least a day before the procedure either in person or by calling 843-777-2095 (McLeod) or 843-661-3047 (Carolinas) with insurance information in hand.  3.Do not eat or drink anything after midnight the evening before the procedure.  4.Someone else must be able to drive you home after sedation for the  procedure since it is illegal to drive under these conditions for 24  hours. The procedure will not be done unless this arrangement is made.You are not allowed to ride a bus unless accompanied by someone. 5.If you take Coumadin or warfarin, stop taking it 4 days before the procedure (if ok with your physician) and be sure to tell the person at check-in so a prothrombin time can be drawn on day of procedure. 6.You may take your Blood Pressure and Heart medications with a little water on the morning of the procedure. If diabetic, do not take diabetic pills or insulin before procedure.
